In the 2008-2009 financial year, Country Health SA - Central Operations, Healthy Communities Portfolio has made available $5,000 to each SPOKE network for Primary Health Care small grants. These funds are available to local organisations/agencies and community groups to fund small Primary Health Care / Health Promotion activities/projects. The funding will be administered by the auspicing body, Wakefield SPOKE Inc wit the funds being accessed via the local SPOKE's through an application process. In the first instance please contact your Grant Liaison Officer to help develop your idea and the application.

CRITERA FOR APPROVAL

• Requests must identify that the project relates to an identified health / wellbeing / community issue or need
• Requests need to describe the project and the activities for which the funds will be used
• Strategies must take into consideration the Primary Health Care Principles. Your grant liaison officer can assist you to develop your application
• Requests for funding must be received one week prior to SPOKE meeting for consideration at that meeting
• Grants maybe used as seed funding to attract other funds and in-kind contributions involving partners
• you are encouraged to link up and involved service clubs, other community groups, businesses, councils etc

Grassroots community groups that have difficulty obtaining funds are a priority, rather than groups who receive recurring or government funding

• Funding for up to $500 available
• Requests for funding must use the application form (available for downloading below)
• Applicants must provide a brief evaluation within 2 months of completing the activity / project
• Grants are for 'one off' projects only, not recurrent projects and not projects funded previously
• If your group does not have an ABN number, your grant can be held (on your behalf) by a sponsoring body
